At True Impact Foundation™, we understand that housing is more than shelter—it is a stabilizing force that directly impacts health, safety, and long-term independence. The Next Chapter House was created with this understanding at its core.
Why Housing Stability Matters
Stable housing provides the foundation individuals need to heal, rebuild, and move forward. Without it, people are more likely to experience repeated crises that strain families, healthcare systems, and communities. Transitional housing bridges the gap between instability and independence by offering a safe, structured environment where residents can regain footing and plan their next steps.

Supporting Health & Preventing Re-Hospitalization
For hospitals, discharge planners, and healthcare partners, safe housing is a critical component of successful post-discharge outcomes. Individuals released from hospitals without stable housing face higher risks of complications, medication non-compliance, missed follow-up care, and avoidable re-hospitalization. The Next Chapter House provides a calm, supportive environment that helps individuals transition safely back into the community. By offering stable housing paired with structure and external service coordination, we help reduce preventable returns to emergency rooms and inpatient settings—supporting continuity of care beyond the hospital walls.
Reentry, Reintegration, and Reduced Recidivism
For individuals rebuilding after justice involvement, housing stability is one of the strongest predictors of successful reintegration. Without it, the risk of recidivism increases significantly. Our transitional housing model supports accountability, routine, and community connection—key elements that help individuals move forward with confidence, purpose, and responsibility. By addressing housing as a foundational need, we help remove barriers that often lead to repeated system involvement.
A Commitment to Accessibility & Inclusive Housing
As part of our long-term vision, True Impact Foundation™ is actively developing plans for fully ADA-compliant, wheelchair-accessible housing. This expansion is designed to better serve individuals with disabilities, mobility challenges, chronic conditions, and those transitioning from hospitals or skilled nursing facilities. Accessible housing is not optional—it is essential. As communities age and healthcare needs become more complex, the demand for housing that supports physical accessibility, safety, and independence continues to grow. Our commitment reflects a forward-looking approach that prioritizes equity, dignity, and practical solutions for real-world needs.

The Next Chapter House is not a stand-alone solution. We collaborate with hospitals, community organizations, service providers, and funders to strengthen outcomes and maximize impact.
